The mix‑up caused the family to believe Joshua survived, leading to a tragic miscommunication. SYP has referred itself to the Independent Office for Police Conduct (IOPC) after forensic checks confirmed the error. An 18‑year‑old was arrested on suspicion of dangerous driving, and a 19‑year‑old on suspicion of perverting the course of justice. The investigation aims to prevent future incidents.
